How do I book an appointment to see a psychiatrist?
To see a psychiatrist will require a referral is sent by your family physician or nurse practitioner to Dr. Tariq M. Hassan through Ocean referral (Summit NeuroCare) or our confidential and secure fax number 613-817-4331.
If you are currently under the care of a psychiatrist who is retiring or leaving, please contact the hospital/clinic where you are being seen for information as to who will be taking over you care.
We kindly request that you do not fax any personal information to this number as this will be deleted.
The confidential fax number is only to be used by healthcare providers.

Missed Appointment Policy
24 hours’ notice required for cancellations
Missed or late-cancelled consultation (first-time) appointments may be subject to a $150 fee
Fees are not covered by OHIP
This policy follows guidance from the College of Physicians and Surgeons of Ontario and the Ontario Medical Association, which recommend that missed appointment fees be reasonable and clearly communicated in advance.
Exceptions may be considered for emergencies.
